you can fill out these claim forms, you must install the ACORD Viewer.
To download to your machine, click on the image at the right. When the File Download window appears, select Run and follow the directions as
they are presented on screen.
If you need help, please call us at 800-474-5553 or 631-424-2600. We can also send you these forms as PDF files, however, you cannot save or
email completed PDF forms with the full version of Adobe Acrobat. The free Adobe Reader software will only allow you to fill out the form and print it.
